What is Chronic Disease Management?  

Chronic disease is a long-lasting condition with persistent effects, generally lasting more than three months (unlike an acute illness, such as an infection or injury).  These conditions have a complex causality,  long development period, prolonged course of illness and associate functional impairment or disability.

Examples of chronic diseases include:

  • Musculoskeletal conditions (such as arthritis, chronic pain, osteoporosis)
  • Respiratory disease (such as asthma, COPD)
  • Cancer
  • Cardiovascular disease (such as heart disease, hypertension, kidney disease, stroke)
  • Diabetes

  • Autoimmune conditions (such as Crohn disease, rheumatoid arthritis)
  • Obesity & weight control

Chronic disease management is the coordinated care between individuals and their health care providers for the ongoing treatment, management and prevention of disease complications.
